What we do and Service InformationCockburn Community Care provides a range of support services for the frail aged, aboriginal and younger people with disabilities.
Cockburn Community Care follow the Wellness Principles with a strong emphasis on promoting independence, and are committed to developing individual options to meet a variety of community needs.
Programs offered are:
Centre-Based Programs provide a wide and varied range of activities and outings that cater to the physical, social and emotional needs of all members.
Lunch and refreshments are included in the days programs. Transport is also provided to and from the Centre in specifically equipped vehicles to allow easy access for people with physical disabilities.
Separate programs are developed for younger people with disabilities, the aged and people with dementia. We also have an Aboriginal specific program which runs culturally appropriate activities.
The Home Support Services assists people to remain living in their own home in the community.

The Community Aged Care Packages are a Commonweath Government funded scheme.
These are planned and coordinated packages of care to assist people to remain living in their own home.
The packages are specifically designed to cater to the individual and are flexible to meet varying needs eg:
To be eligible to receive a Care Package, the client must be assessed by a hospital based Aged Care Assessment Team (ACAT). A referral for this must be obtained from your doctor.
Are you a Carer?
A carer is someone who provides care and support for a parent, partner, child, relative or friend, who has a disability, is frail aged or who has chronic mental or physical illness.
Carers are a valued resource in the community providing most of the necessary in-home care.
To support Carers we offer the following:
The transport program supports existing clients of Cockburn Community Care to attend medical appointments. Eg: hospital, doctors, dentists, podiatry etc.
There is a small fee involved for this service.
To be eligible for services you must have an ongoing functional disability and must reside within the City of Cockburn. Costs for these services are minimal. Fees may vary in accordance with the National Fees Schedule.
The service is based at the Jean Willis Centre, Corner of Healy Road and Ingram Street Hamilton Hill.
The main office is staffed for community contact between 8.00am and 5.00pm Monday to Friday.
