Cockburn Youth Services receives funding from the
Department of Communities Child Protection and Family Support for young people aged 10 to 18 who live, work or study in the Cockburn/ Fremantle area. The Cockburn Youth Outreach team aims to reduce the overall level of a young person’s risk, assist them to improve relationships with family where required and improving a young person’s connection with support services as required.
The Youth Outreach team facilitates this through case management, a collaborative process of assessment, planning, care, evaluation and advocacy for options and services to meet individual and family needs.
Once a young person is connected with the program, their support worker will encourage them to talk about issues they are facing including:
- Family conflict
- Difficulties at school
- Accommodation
- Relationships
- Drugs and alcohol
- Sexual health
- Education
- Employment
- Training
- Other related issues
The young person and their support worker will work together to deal with the issues they are experience. This includes providing support around behavioural change, education support and referral to relevant support services.
If you have a young person who could benefit from the support of one of our Youth Outreach Workers, you can refer them to our service. Referring a young person to our service is simple:
- First, speak with the young person and guardians about the service to obtain their consent. We find young people who want to work with us and see the benefits in engaging with the Outreach team receive the best outcomes for themselves.
- Complete the Support Services Referral Form, found below.
- Return the form via email to our Youth Outreach Workers.
- One of our Youth Outreach workers will make contact with the referrer to discuss the referral.
- The Youth Outreach worker will then contact the family to set up an initial meeting to connect.
This service is
free to access and offered to all young people who meet the service’s criteria
