Request to hire a City park or reserve for group fitness classes

This form is to be completed by personal trainers wishing to register interest to conduct outdoor group fitness or private sessions (including yoga/Pilates) on any parks or reserves owned by the City of Cockburn.

Please note:   We request that PTs use only small parks or passive reserves for their training sessions (ie those not used for cricket, football, rugby, softball, etc).

Do you have the following:

Public Liability Insurance (minimum $10 million cover)?
Appropriate fitness accreditation?
An up to date National Police Clearance Certificate?
Membership to AusActive (formerly Fitness Australia)?
