Please note: If you are transferring INTO the City of Cockburn from an external council, please complete the New Dog Registration Form or the New Cat Registration Form instead.

If your animal has passed away, had a change of owner, change of address, a new microchip number, been sterilised or requires a replacement registration tag, complete the form below.

All fields must be completed unless indicated as optional. Your owner information needs to be kept up-to-date.
